Unveiling Insights: A Conversation with Dr. Pallav Mehta on Breast Cancer

The pursuit of understanding and treating breast cancer is an ongoing journey, marked by advancements and challenges alike. To shed light on this critical field during Breast Cancer Awareness Month, Reimagine Care sat down with Medical Director, Dr. Pallav Mehta, a distinguished expert in breast cancer, to delve into his insights, experiences, and the evolving landscape of breast cancer research and treatment.

Thank you, Dr. Mehta, for joining us today. To kick things off, could you share a bit about your background and what drew you to specialize in breast cancer?

Dr. Mehta: My journey in oncology began with a deep interest in understanding complex diseases and a genuine desire to make a meaningful impact. No one forgets their cancer diagnosis and I always consider it a privilege to be involved during such an important time in the lives of patients and their families.

Breast cancer, being one of the most prevalent cancers affecting women worldwide, presented an opportunity to contribute significantly to both research and patient care. The challenges and nuances of breast cancer drew me in, and I’ve been dedicated to this field ever since.  

I remember about 25 years ago during medical school I was tagging along on rounds with a team of 10 or more physicians, all far more experienced than me. We met a young woman with stage IV breast cancer, whose husband was at the bedside, and we learned she herself was a physician and a mother of two young children. After going through the usual panoply of questions, she told us that she and her husband finally told their 6-year-old daughter about her diagnosis. The response from her daughter was “But…if you’re not here, then, then who is going to get the Oreos for me from top shelf?” It was a simple moment that had a profound impact as it taught me about our individual value to other people in our lives and how this disease can so unceremoniously rip it away. That woman, however, was placed on at that time, an experimental new drug that led to her cancer melting away. It was the dawn of the targeted era of cancer treatment, and I was hooked.

Given the rapidly evolving landscape of cancer research, what recent developments or breakthroughs in breast cancer treatment have caught your attention?  

Dr. Mehta: We’re witnessing remarkable progress in breast cancer treatment, especially in the realm of targeted therapies and immunotherapy. Personalized medicine has become a key focus, tailoring treatments based on the unique genetic makeup of each patient’s tumor. Additionally, advancements in early detection methods, such as liquid biopsies, hold great promise in identifying cancer at its earliest stages, even potentially using these tests at some point as a screening tool in addition to mammograms, for example.

How do you see the role of immunotherapy in breast cancer evolving, and what potential benefits does it offer for patients?

Dr. Mehta: Immunotherapy has revolutionized cancer treatment across various malignancies, and breast cancer is no exception. Ever since the modern version of cancer treatment began after WWII, we have focused on treatments that try to kill the cancer, often at the expense of some harrowing side effects to normal healthy cells. But now, not only are those treatments becoming more and more precise, we are also for the first time, teaching/activating the immune system to find and kill the cancer.

In your experience, what are some of the challenges that patients and healthcare providers face in the realm of breast cancer, and how can these be addressed?

Dr. Mehta: One persistent challenge is late-stage diagnosis. While we have made many advances over the last quarter century in the stage IV diagnosis, with many treatments available, for most patients these are not curative and eventually they succumb to their disease. Increasing awareness about breast health, promoting regular screenings, and improving access to healthcare resources are essential in addressing this issue. Another challenge lies in the disparities in cancer care, with some communities facing barriers to timely and quality treatment. Efforts to reduce these disparities, both at the systemic and individual levels, are crucial for achieving equitable outcomes.

Looking ahead, what do you envision as the future of breast cancer, and what advancements or trends do you anticipate?

Dr. Mehta: The future of breast cancer oncology holds exciting prospects. We can expect continued advancements in precision medicine, with therapies becoming even more targeted and effective, often combining various approaches in one large plan of care. Artificial intelligence and machine learning will likely play a significant role in data analysis, helping us uncover patterns and potential treatment strategies. Collaboration between researchers, clinicians, and industry partners will be key to driving innovation and bringing novel therapies to patients.

I also expect to see a focus on the improvement of quality of life during and after treatment, rather than just extension of life. We are learning that it’s important not just to treat the patient, but to care for the patient. This synergy promises to redefine cancer care, making it more efficient, patient-centric, and ultimately more effective providing an enhanced cancer journey for patients and providers alike.
